Online Request Form

To start the process of ordering through Stitchin 4 U, follow these 3 simple steps to fill out our request form. Once the information is submitted a representative will contact you to complete your order.

Step 1. Choose your apparel

Visit our Apparel and Accessories page. To view our products click on one of the provided links to browse our online catalogs. When you find the items you are looking for type the style #, color, qty and size in the request form.

Step 2. Pick out your design or submit your own custom design

Visit our Designs page. When you choose a design, type the design number in the design section of the request form. If you already have a custom design upload a jpeg or vector file of your design or logo along with the size you would like it and we can give you a quote for the cost to create the design in stitches.

Step 3. Lettering

If you would like lettering please fill out the lettering section of the request form.
